29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.  a)  On 07-21-14, employee working from six foot step ladder on an approximately 3 ft. W x 8 ft. H exterior wall opening without an adequate fall protection system; wall opening outside bottom edge greater than ten feet above first story roof surface and inside bottom edge approximately 12 inches above working surface/rung.

29 CFR 1926.502(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge:  a)  On 07-21-14, employee who was working from a stepladder on a wall opening had installed one approximately 2 in. W x 4 in. D x 48 in. L board in the wall opening on the outside exterior brick using two 3 in. L screws; the board was approximately 36 in. above the working surface/rung.

D x 48 in. L board was installed to the outside surface of exterior wall with two decking screws as fall protection in an approx. 3 ft. W x 8 ft. H wall opening.
