29 CFR 1910.132(a): Protective equipment was not used when necessary when a hazard of process or environment capable of causing injury or impairment of the body through physical contact was encountered. Employees working on the drilling rig were exposed to injuries in that personal protective equipment was not always available such as: a) Hard hats were not worn during drilling operations. b) Fire resistant (FR) clothing was not provided to employees working on the rig.

29 CFR 1910.151(b): In the absence of an infirmary, clinic, or hospital in near proximity to the workplace which is used for the treatment of all injured employees, a person or persons were not adequately trained to render First Aid. Employees did not have prompt access to treatment of injuries in that employees were not provided first aid training nor was a hospital or clinic located near the jobsite.
