29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose. a) Shop Area, 164 Seneca Road, Lehighton, PA - Employees operated a Reed Injection Molding Machine (Machine #10) to manufacture plastic parts with conductors for the solenoid valves on the north side of the machine that were energized (120V) and uncovered. Employees were exposed to a contact with electrical current hazard, as observed on or about November 20, 2014. In accordance with 29 CFR 1903.19(c), abatement certification is not required for this violation. The hazardous condition was corrected during the inspection.

29 CFR 1910.36(e)(2): The door that connects any room to an exit route must swing out in the direction of exit travel if the room is designed to be occupied by more than 50 people or if the room is a high hazard area (i.e., contains contents that are likely to burn with extreme rapidity or explode). a) Southwest Wall near Machine #10, 164 Seneca Road, Lehighton, PA - The employer had constructed and attached a wooden door frame with an inward swinging screen door to an emergency exit metal door frame with an outward swinging metal door. Employees who chose to vacate the building by this emergency exit in the event of fire were exposed to smoke inhalation and fire hazards, as observed on or about November 20, 2014. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

Employees who chose to attempt to extinguish a fire with the portable fire extinguishers were exposed to fire hazards, as observed on or about November 20, 2014. 29 CFR 1910.304(f)(1)(iv): Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel. a) Southeast wall behind injection molding machine #13, 164 Seneca Road, Lehighton, PA - Employee access to the overcurrent device (electrical panel) for electrical equipment, such as but not limited to an abrasive wheel machine and a feed hopper system for an injection molding machine, was blocked by an abrasive wheel grinder machine. Employees were exposed to slips, trips and falls and the hazards associated with being unable to de-energize equipment in the event of an emergency, as observed on or about July 3, 2014. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
