29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  On or about November 20, 2014, at 3320 NW 84th CT 34 Dr. employees were cutting materials and performing other construction work with cords, raw and cut lumber and other debris scattered throughout the work area, exposing employees to the hazard of slips, trips and falls.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about 20 November, 2014, at 8329 NW 34 Dr, employees were performing concrete work off a deck of a residence without the use of fall protection, exposing the workers to a 20-foot fall hazard.  Fast Carpentry 1 was previously cited for a Repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926(501)(b)(11) , which was contained in OSHA inspection number 836263, citation number 2, item 1 and was affirmed as a final order on 3/25/2013, with respect to a workplace located at 12550 W Sunrise Blvd Sunrise, FL 33323.
