29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest system.  On or about November 19, 2014,  employees who were performing masonry activities on the roof of a newly constructed residential home were not provided fall protection exposing the employees to an unprotected fall hazard of over 6 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about November 19, 2014, the employer did not ensure employees performing masonry activities on the roof of a residential home were trained to recognize and avoid fall hazards and the procedures to be followed in order to minimize or avoid those hazards.
