29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about November 20, 2014, an employee operated a Hitachi nail gun to perform roofing activities without eye and face protection exposing him to the hazard of being struck-by flying objects.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest. On or about November 20, 2014, employees who were installing roofing material at a height of over 6 feet were not provided fall protection. Employees were exposed to an unprotected fall of over 12 feet to the ground below.
