29 CFR 1917.45(e)(1): Exposed moving parts, such as gears, chains, and chain sprockets, that present a hazard to employees during crane and derrick operations, were not securely guarded.  a) Smoky Mountain Transfer - On or about 11/20/14, employees were exposed to amputation hazards when the guard had been removed from the crane body.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1917.45(i)(2): Accessible areas within the swing radius of the body of a revolving crane were not physically guarded during operations to prevent an employee from being caught between the body of the crane and any fixed structure or between parts of the crane.  a) Smoky Mountain Transfer - On or about 11/20/14 - employees were exposed to struck-by hazards when the swing radius of the crane was not barricaded.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
