29 CFR 1918.32(b): The edge of a hatch section, or of stowed cargo, was not guarded by a vertical safety net or other means providing equal fall protection to exposed employees.  California barge:  On or about November 20, 2014 and times prior thereto, employees were walking the tiers of lumber at heights over 10 feet.  This exposed employees to fall hazard to the deck.

29 CFR 1918.65(h)(11): An employee was elevated by a fork lift truck without the platform being secured to the lifting carriage or forks.  Teevin dock California barge:   On or about November 20, 2014 and times prior thereto, employees would walk across the forks on the elevated load and descend down the ladder or one employee rode the load. This exposed employees to fall hazard from over 10 feet and potential caught-in hazard from mechanical drive system of the forks.
