29 CFR 1910.157(c)(1): The employer provided portable fire extinguishers, but did not mount, locate and identify them so that they are were readily accessible to employees without subjecting the employees to possible injury: 7 Depot St., South Grafton, MA: On or about November 24, 2014, the following issues were observed with the placement of employer provided portable fire extinguishers: a) A portable fire extinguisher on an outside wall was blocked by wood storage and machinery. b) A portable fire extinguisher adjacent to the roll up door was hidden behind wood and rolling carts. c) The location that was wall marked as a fire extinguisher location by the entry area was lacking a fire extinguisher.

29 CFR 1910.213(a)(15): Combs (feather boards) or suitable jigs were not provided at the workplace for use when a standard guard cannot be used, as in dadoing, grooving, jointing, moulding, and rabbeting: a) 7 Depot St., South Grafton, MA: On or about September 22, 2014, an employee suffered lacerations to his left hand when he was cutting dados on the Powermatic 10" Table Saw at this location without any of the equipment mandated when the standard guard has to be removed to perform certain cuts on the table saw. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
