29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:   Audiometric testing was not provided to all employees whose exposures equaled or exceeded an 8-hour time-weighted average of 85 decibels.   (1)  An employee working on a chop saw was exposed to continuous noise levels at 105% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent sound level of approximately 90.3 dBA.  The sampling was performed for 360 minutes during one shift on December 4, 2014; exposure calculations included a zero increment for the 120 minutes not sampled.  (2)  An employee working on a chop saw was exposed to continuous noise levels at 94.2% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent sound level of approximately 89.5 dBA.  The sampling was performed for 363 minutes during one shift on December 4, 2014; exposure calculations included a zero increment for the 117 minutes not sampled.
