29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  Jobsite located at 152 N. Terrace, Wichita, KS 67208; employees were exposed to electric shock hazards in that the strain relief was pulled out on the female end of the 115 volt extension cord used to provide power for tools at the jobsite.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   Jobsite located at 153 N. Terrace, Wichita, KS 67208; employees engaged in residential roofing work on a roof with pitch up to 14 in 12, were exposed to fall hazards of up to 9 feet, 8 inches from the eave to the ground.  Fall protection was present at the jobsite, but was not used.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Jobsite located at 152 N. Terrace, Wichita, KS  67208; employees working at heights greater than 6 feet were not adequately trained to recognize and minimize fall hazards.
