29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) November 25, 2014 and at times prior thereto; upper platform/Northside; employees framing a new residential home were observed walking the trusses and working on the outside wall without a personal fall arrest system in place to prevent them from falling an estimated 18 feet to ground level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. a) November 25, 2014 and at times prior thereto; upper platform/north-side; the extension ladder and step ladder used to access the working platform were not properly extended above the working platform. The extension ladder was 12 inches below the platform and the step ladder located within the expansion room was 48 inches below the working platform.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step. a) November 25, 2014 and at times prior thereto; upper platform/north-side; the six foot step ladders used to access the roof area were not tall enough to extend three feet which resulted in the employees using the top step while accessing the working platform or while stretching from the platform area to access the top of the ladder; exposing employees to a fall hazard of 10 feet to the second floor platform.
