29 CFR 1910.95(b)(1): When employees were subjected to sound exceeding those listed in Table G-16, feasible administrative or engineering controls were not utilized:    On or about August 13, 2013, a dosimeter (sn: NLH090055) was placed on an employee working in the grinding area for a total of 433 minutes. The employee was exposed to noise at 888.9% of the permissible daily noise exposure level, with an equivalent time-weighted average sound level of 105.7 dBA. Exposure calculations include a zero increment for the 47 minutes not sampled.     Quaker City Castings, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1910.95(b)(1), which was contained in OSHA inspection number 916302, citation number 1, item number 1a, and was affirmed as a final order on November 12, 2013, with respect to a workplace located at 310 East Euclid Street, Salem, OH, 44460.    Controls usually applicable in these circumstances include but are not limited to:    1. Installation of mufflers on the grinders to reduce noise from the exhaust ports    2. Replacement of air powered grinders with electric powered tools    3. Separation of workers by distance or through the use of partitions and  soundproofing surfaces so that employees do not receive noise from adjacent workers     4. Rotation of employees to quieter areas to  limit their daily exposure to noise.    Step 1: Submit to the Area Director a written detailed plan of abatement outlining a schedule for the implementation of engineering and/or administrative measure to control employee exposure to noise as referenced in this citation. This plan shall include, at a minimum, target dates for the following actions which must be consistent with the dates required by this citation.       1. Evaluation of engineering control options     2. Selection of optimum control method and completion of design       3. Procurement, installation, and operation of selected control measures       4. Testing and acceptance or modification/redesign of controls       Step 2: All proposed control measures shall be evaluated for each particular use by a competent industrial hygienist or other technically qualified person. Thirty day progress reports are required during the abatement period. Abatement must be completed by 30 days.       Step 3: Abatement should be completed by the implementation of feasible engineering and/or administrative controls. Verification must be provided, and should include noise monitoring results showing levels of noise below the permissible exposure limit. Abatement must be completed by 60 days.

29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of copper fume and iron oxide fume listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentrations of 0.1 mg/m3 and 10 mg/m3, respectively.         a. On or about December 11, 2014, an employee working in the air arc booth was exposed to copper fume at a time weighted average of 0.583 mg/m3, which is above the permissible exposure limit of 0.1 mg/m3.      b. On or about December 11, 2014, an employee working in the air arc booth was exposed to iron oxide fume at a time weighted average of 32.588 mg/m3, which is above the permissible exposure limit of 10.0 mg/m3.    Quaker City Castings, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1000(a)(2), which was contained in OSHA inspection number 980029, citation number 1, item number 2a, and was affirmed as a final order on September 4, 2014, with respect to a workplace located at 310 East Euclid Street, Salem, OH, 44460.

29 CFR 1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d):    a. On or about July 2, 2014, an employee working in the air arc booth was exposed to copper fume at a time weighted average of 0.335 mg/m3, which is above the permissible exposure limit of 0.1 mg/m3.      b. On or about July 2, 2014, an employee working in the air arc booth was exposed to iron oxide fume at a time weighted average of 29.733 mg/m3, which is above the permissible exposure limit of 10.0 mg/m3.      Ventilation that was present was not appropriately designed to reduce employee exposure.     Quaker City Castings, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1000(e), which was contained in OSHA inspection number 980029, citation number 1, item number 2b, and was affirmed as a final order on September 4, 2014, with respect to a workplace located at 310 East Euclid Street, Salem, OH, 44460.
