29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    (a) Location - 164 Holland Street, Somerville, MA:    On or about 12/1/2014, the ladder used by an employee to access the entry way roof did not extend at least three feet past the roof and was not secured at its top.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    (a) Location - 164 Holland Street, Somerville, MA:    On or about 12/1/2014, employees on a ladder jack scaffold greater than ten feet above the ground were not protected from falling by a personal fall arrest system.    Joseph S. Savini, Incorporated was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 901942, citation number 1, item number 2 and was affirmed as a final order on 8/14/2014, with respect to a workplace located at 9 Bell Rock Road, Malden, MA 02148.
