29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  Location - 6 Landers Road, Stoneham MA  On 7/18/2023, one worker without head protection was observed picking up construction debris directly below where roofers were installing new shingles, placing him at risk of head injuries from falling objects or debris.  Instance A: One worker was observed collecting discarded roofing shingles using a large shovel and placing them into a wheelbarrow. The worker wore a fishing-style cloth hat and was positioned directly beneath others who were actively installing roofing materials.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).  Location - 6 Landers Road, Stoneham MA  On 7/18/2023, three employees working on a steep pitched roof, at heights ranging from 9'4" to > 16', were not protected from falls. Many employees were wearing fall protection harnesses which were not connected to any anchorage points or lifelines. Roof scaffold brackets were being used to support workers and materials, but this system does not replace properly configured fall protection.  Instance A: Two workers were working on a steep pitched roof installing a weatherproofing membrane and shingles without properly configured fall protection. Both workers were not wearing a fall protection harness and can be seen standing on a roof scaffold bracket system.  Instance B: One worker was nailing new shingles into place with a hammer while wearing a fall protection harness but not connected to an anchor point.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  Location - 6 Landers Road, Stoneham MA  On 7/18/2023, two workers were exposed to fall hazards by accessing the roof using ladders whose side rails did not extend at least 3 feet above the upper landing surface.   Instance A: An access ladder to a ladder jack scaffold was in place with less than 3 feet extending above the landing surface on the scaffold. Instance B: An access ladder to the roof was in place with less than 3 feet extending above the roof.
