29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): 150 Grove St, Worcester, MA On 11/4/14, employees operated the Lauderdale-Hamilton panel saw without the adjustable guards completely enclosing the point of operation, resulting in a thumb amputation injury at the point of operation. Rand-Whitney Packaging Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 538078, citation number 1, item number 1a and was affirmed as a final order on October 19, 2012, with respect to a workplace located at 150 Grove St, Worcester, MA.
