29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Lower Building, Draw Bench #5C, #6B & #10B:  the final drawing block on these machines were unguarded, exposing employees to rotating parts, observed on or about November 21, 2014  b) Upper Building, Draw Bench #6A: the final drawing block on this machine was unguarded, exposing employees to rotating parts,observed on or about November 21, 2014  b) Gimax 1 & 2:  Interlock on doors were bypassed, exposing employees to the point of operation, observed on or about November 21, 2014  c) Minicell Draw Bench #6D: the first three blocks have inadequate guards, exposing employees to the rotating parts, observed on or about November 21, 2014  d) Annealer #1, Take Up #8:  unguarded pinch point while stringing up wire, exposing employees to point of operation, observed on or about November, 21, 2014                            ABATEMENT CERTIFICAITON AND DOCUMENTATION REQUIRED FOR THIS VIOLATION.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:  a) 394 Arcos Drive, Mount Carmel, PA 17851: Employer's LO/TO Program did not clearly and specifically outline the steps to take to shut down, isolate, block, or secure each machine such as the 14 Vaughn Draw Benches and the two Gimax Omnispora wire handling machines, on or about November 21, 2014.                                   ABATEMENT CERTIFICATION REQUIRED FOR THIS VIOLATION.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a) 394 Arcos Drive, Mount Carmel, PA 17851:  An annual inspection of the energy control procedures (Lockout/Tagout) was not conducted at least annually by the employer, on or about  November 21, 2014.                                               ABATEMENT CERTIFICATION REQUIRED FOR THIS VIOLATION.
