29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  a) The employer does not ensure that each working level of a scaffold is fully planked or decked between the front uprights and the guardrail supports.  This violation was most recently observed at 1325-1327 North 15th Street, Philadelphia, PA on November 25, 2014 where employees were performing bricklaying operations up to approximately thirty-six (36) feet above the ground from a fabricated frame scaffold that was not fully planked or decked.  To abate this ongoing violation, the employer must install decking or planking from the front uprights to the guardrail supports on all working levels.  McGee Plastering and Stucco Inc. was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection 486658, citation 1, item number 2a, and was affirmed as a final order on May 6, 2013 with respect to a workplace located at Hope and Hancock Streets, Philadelphia, PA.  McGee Plastering and Stucco Inc. was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 895701, citation number 2, item number 1 and was affirmed as a final order on April 9, 2013 with respect to a workplace located at 12th and Latona Streets, Philadelphia, PA.  McGee Plastering and Stucco Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 315819383, citation number 1, item number 1 and was affirmed as a final order on June 8, 2012 with respect to a workplace located at 8th and Christian Streets, Philadelphia, PA.  McGee Plastering and Stucco Inc. was previously cited for a violation of this Occupational Safety and Health standard which was contained in OSHA Inspection Number, 314236092, Citation Number 1, Item Number 1 and was affirmed as a final order on February 27, 2012 with respect to a workplace located at Broad & Catherine Streets, Philadelphia, PA. 29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) are not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph.  a) The employer does not restrain the fabricated frame scaffolding from tipping by guying, tying, bracing or equivalent means.  This violation was most recently observed on the front wall of 1325-1327 North 15th Street, Philadelphia, PA on November 25, 2014 where employees were performing bricklaying operations from a fabricated frame scaffold that was not restrained from tipping by installing guys, ties, and braces.  The fabricated frame scaffold was approximately thirty-six (36) feet in height and approximately five (5) feet in width thus the height to base ratio was seven to one (7:1).  To abate this ongoing violation, the employer must require the use of guying, tying, bracing, or equivalent means when the height to base ratio exceeds four to one (4:1) on supported scaffolds.  McGee Plastering and Stucco, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(c)(1) which was contained in OSHA inspection number 906301, citation number 2, item number 2 and was affirmed as a final order on December 5, 2013 with respect to a workplace located at 274 Leverington Avenue, Philadelphia, PA. 29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:     a) When scaffold platforms are more than 2 feet above or below a point of access, the employer does not use ladders, stair towers, or other acceptable means of accessing the scaffold platforms.  This violation was most recently observed at the property 1325 - 1327 North 15th Street, Philadelphia, PA on November 25, 2014 where employees were performing bricklaying operations from a fabricated frame scaffold that had no ladders, stair towers, or other safe means of access.  The fabricated frame scaffold was approximately thirty-six (36) feet in height and employees were lowering themselves onto the outrigger platform from an open window approximately six (6) feet above the working scaffold platform.  To abate this ongoing violation, the employer must require the use of portable ladders, attachable ladders or equivalent safe means of access.  McGee Plastering & Stucco Inc. was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 906301, citation number 3, item number 1 and was affirmed as a final order on December 5, 2013 with respect to a workplace located at 274 Leverington Avenue, Philadelphia, PA. 29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a) The employer does not require employees to use any form of personal fall arrest systems or the use of guardrail systems while performing bricklaying operations from a fabricated frame scaffold.  This violation was most recently observed at the property 1325 - 1327 North 15th Street, Philadelphia, PA on November 25, 2014, where employees performing bricklaying operations from the outrigger portion of the 6th level of the tubular welded frame scaffold were exposed to fall hazards of approximately thirty-six (36) feet without the use of personal fall arrest systems or guardrail systems.  To abate this ongoing violation, the employer must provide and require the use of personal fall arrest systems or guardrail systems while employees are utilizing the working levels of the scaffold.  McGee Plastering and Stucco Inc. was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 314230426, citation number 2, item number 3 and was affirmed as a final order on January 25, 2011 with respect to a workplace located at 224-234 George Street, Philadelphia, PA.
