29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:    a) On or about December 3, 2014, the portable fan set on the stair landing next to the Paper Machine #2 Wire Section Dry Side created a tripping hazard.    b) On or about December 3, 2014, the extension cord for the portable fan on the Paper Machine #2 Wet End Supple Fan Motor inspection platform located in the Air Cap Burner Room created a tripping hazard.    c) On or about December 3, 2014, the power and extension cords for the 2 portable fans on the mezzanine above the Paper Machine #2 Head Box created a tripping hazard.

29 CFR 1910.23(a)(8): Floor holes, which persons could accidentally walk into, were not guarded by standard railings with standard toeboards on all exposed sides or by floor hole covers of standard strength and construction:    a) On or about December 3, 2014, a floor hole and corroded grating on the Paper Machine #2 Catwalk along the Wire Section Dry Side was guarded by caution tape for over 2 months. Caution tape does not meet the requirements for a standard guard rail.
