29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:  a.  Located on the eastern side of a newly constructed masonry building, an employee was performing masonry work from the top of the second buck of a Waco walk-through tubular weld frame scaffold that was not fully planked, thereby exposing the employee to a fall hazard of approximately 14 feet.   b.  Located on the eastern side of a newly constructed masonry building, an employee was performing masonry work from the top of the first buck of a Waco walk-through tubular weld frame scaffold that was not fully planked, thereby exposing the employee to a fall hazard of approximately 7 feet.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.  a.  Located on the eastern end of a new masonry construction building located at 321 South Hamilton Road in Gahanna, Ohio, an employee was gaining access to the top to of the second buck on a walk-through tubular weld frame scaffold by means of climbing a cross brace, thereby exposing the employee to a fall hazard of approximately 12 feet.   a.  Located on the western end of a new masonry construction building located at 321 South Hamilton Road in Gahanna, Ohio, employees were gaining access to the first and second buck platforms by means of climbing a Waco walk-through tubular weld frame scaffold.  The scaffold was not a safe means of access, thereby exposing the employees to a maximum fall hazard of approximately 14 feet.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.  a.  Located on the eastern-portion of the new construction building at 321 South Hamilton Road in Gahanna, Ohio, employees were performing masonry work from the top of the second buck of a walk-through tubular weld frame scaffold without any means to protect the employees from a fall, thereby exposing the employees to an approximate 14 foot fall hazard.    b.  Located on the western-portion of the new construction building at 321 South Hamilton Road in Gahanna, Ohio, employees were performing masonry work from the top of the second buck of a walk-through tubular weld frame scaffold without any means to protect the employees from a fall, thereby exposing the employees to an approximate 14 foot fall hazard.

29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).  a.  Located at 321 South Hamilton Road in Gahanna, Ohio, an employee was operating a JLG Skypower 450AJ aerial lift without the use of fall protection, there by exposing the employee to a fall hazard of approximately 12 feet.

29 CFR 1926.602(c)(1)(vi): All industrial trucks in use shall meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks.   a.  Located on the eastern side of the new masonry construction building located at 321 South Hamilton Road in Gahanna, Ohio, an employee was exposed to a struck-by hazard from walking on the second buck of scaffolding while under a forklift loaded with scaffold planks.
