29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    (a) Jobsite, Lowe's, West Mifflin, PA:  On or about July 18, 2016 the working level of the scaffold on the south side of the building was not fully planked or decked.  An employee was standing on one plank and unloading brick from a powered industrial truck and placing it another plank.  The fall hazard was approximately 14 feet to the ground below.      The employer, A&V Masonry, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(1), which was contained in OSHA inspection number 1011493, citation number 1, item number 1 and was affirmed as a final order on 03/18/2015, with respect to a workplace located at 321 South Hamilton Road, Gahanna, OH.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces shall not be used as a means of access:    (a) Jobsite, Lowe's, West Mifflin, PA:  On or about July 18, 2016 there were no portable ladders or other means of access used on the scaffold(s) erected at the site.  Employees climbed the cross-bracing and frame ends to move between the working levels exposing them to fall hazards up to approximately 14 feet.      The employer, A&V Masonry LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(e)(1) which was contained in OSHA inspection number 1011493, citation number 1, item number 2 and was affirmed as a final order on 03/18/2015 , with respect to a workplace located at 321 South Hamilton Road, Gahanna, OH.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    (a) Jobsite, Lowe's, West Mifflin, PA:  On or about July 18, 2016 employees were working on the 2nd level of the walk-through tubular scaffold and no fall protection systems, such as guardrails or personal fall arrest systems, were in place exposing employees to fall hazards of approximately 14 feet.      The employer, A&V Masonry, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1), which was contained in OSHA inspection number 1011493, citation number 1, item number 3 and was affirmed as a final order on 03/18/2015, with respect to a workplace located at 321 South Hamilton Road, Gahanna, OH.
