29 CFR 1910.37(a)(3):   Exit routes were not continuously maintained free of obstructions or impediments to full instant use in the case of fire or other emergency:   a/ J & M Futon/Edison NJ Site: Warehouse. Exit doors were blocked by boxes of product, cardboard boxes of miscellaneous and other material, preventing free escape from the facility in the event of a fire or other emergency (including but not limited to)   Warehouse Shipping/Receiving Dock Area.  1/ Exit door located on the left side of the Shipping/Receiving Dock towards the office was blocked by cardboard boxes of product. 2/ Exit door on the right side of the Shipping/Receiving Dock leading to the yard/empty Propane storage cage was blocked by cardboard boxes of miscellaneous and other material. Observed on/or about 12/04/2014.

29 CFR 1910.178(m)(3):  A safe place to ride was not provided where riding a powered industrial truck(s) was authorized.  a/ J & M Futon/Edison NJ Site: Warehouse (Area adjacent to Shipping/Receiving Dock). An employee was elevated by a forklift operator using a powered industrial truck with a wooden pallet attached to the forklift blades. The employee was standing on the pallet while elevated approx. 8 from the floor (storage rack tier 2 level) without personal fall protection, or an approved cage and without the pallet being secured to the powered industrial truck.  Observed on/or about 12/04/2014.
