29 CFR 1910.23(c)(3): Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards were not guarded with a standard railing and toe board: An employee was exposed to injury from a fall onto the moving casting and mold debris on the shaker table. Standard railing was not provided to prevent a fall from the elevated work platform onto the shaker table.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: Employees working in the maintenance area were exposed to electrical shock hazards in that flexible cords were not equipped with strain relief on the following equipment: a) On the Jet band saw motor. b) On plug end of the flexible cord providing 110 VAC power to the Jet 8 inch grinder.
