Employees were exposed to struck by and amputation hazard.    2) In the Metallurgy Shop area employees using a Broaching Machine that was missing point of operation guarding  to protect employees from the hazards of flying chips and moving parts, exposing employees to a struck by and caught in hazards.

The forklift truck control levers which moves the forks, did not have the control lever knobs marked to identify the function and pattern movement of the controls.
