29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points, rotating parts, and/or flying chips and sparks: Machinery was not provided with point of operation guarding, exposing employees to a caught in and struck by hazard in the following instances: 1) In the Shop Maintenance area employees using a Jet Horizontal Band saw that was missing point of operation guarding. Employees were exposed to struck by and amputation hazard. 2) In the Metallurgy Shop area employees using a Broaching Machine that was missing point of operation guarding to protect employees from the hazards of flying chips and moving parts, exposing employees to a struck by and caught in hazards.

29 CFR 1910.178(a)(6): The user shall see that all nameplates and markings are in place and are maintained in a legible condition. Employees at the Maintenance shop were exposed to struck-by hazards while operating the Mitsubishi forklift truck. Model FG45. Serial Number AF33-00308. Asset 8607 LP Gas Forklift. The forklift truck control levers which moves the forks, did not have the control lever knobs marked to identify the function and pattern movement of the controls.
