29 CFR 1926.452(w)(2): Scaffold casters and wheels shall be locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold is used in a stationary manner. 2432 Albany Ave., West Hartford, CT 06117 on or about 12/02/2014: Two mobile Scaffolds were being used on the second floor and the casters and wheels were not locked as required.

29 CFR 1926.501(b)(14): "Wall openings." Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 m) above the walking/working surface, shall be protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system. 2432 Albany Ave., West Hartford, CT 06117 on or about 12/02/2014: An employee was working on a mobile scaffold near a large wall opening above the top rail and was exposed to 15 foot fall. The employee was not utilizing any type of fall protection.
