29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a) At the worksite, masonry workers laying bricks at 17 feet high were not protected by any type of a fall protection system.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered by experienced and trained employees under the supervision of a competent person:  a) At the worksite, tubular welded frame scaffolding was not erected under the supervision of a competent person in that the person supervising the erection of the scaffold was not familiar with the requirements of 29 CFR 1926 Subpart L.  Employees working from the scaffold, approximately 17 feet above the ground, were exposed to fall hazards.
