29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed. Building 8515, roll-up door number seven; on or about 03 December 2014 and times prior; an energized Kelley Hydraulic Dockboard power control panel had a knockout missing on the right hand side. This condition exposed employees to electrical shock and electrical burn hazard.

Building 8515, battery charging area; on or about 03 December 2014 and times prior; an energized Fellowes 10 foot power tap was substituted for permanent electrical wiring. This condition exposed employees to electrical shock and electrical burn hazard.
