29 CFR 1910.23(c)(2): Runways four feet or more above floor or ground level were not guarded by standard railings (or the equivalent):    The employer is failing to protect employees working at the leased elevator site location of 1st Street, Creston, NE 68631 from fall hazards of 30 feet in that; the runway above the east row of grain bins was not equipped with a standard railing system consisting of an intermediate rails.      Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

The employer is failing to protect employees working at the leased elevator site location of 1st Street, Creston, NE 68631 from fire and explosion hazards resulting from accumulations of up to a 1/2 inch of combustible dust within the elevator leg control room in that; the employer did not implement the written housekeeping program that was developed by the company at the leased site located at 1st Street, Creston, NE 68631.      Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):    The employer is failing to protect employees working at the leased elevator site location of 1st Street, Creston, NE 68631 from entanglement hazards in that; the guards on the screw auger drive pulleys did not fully enclose the pulleys that were located within 32 inches of the ground between the grain storage bins.       Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.    The Cooperative Supply Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.219(d)(1), which was contained in OSHA inspection number 314061409, citation number 1, item number 8a and was affirmed as a final order on August 18, 2011, with respect to a workplace located at 239 Front St, Dodge, NE 68633.

Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.    The Cooperative Supply Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.219(e)(3)(i), which was contained in OSHA inspection number 314061409, citation number 1, item number 8b and was affirmed as a final order on August 18, 2011, with respect to a workplace located at 239 Front St, Dodge, NE 68633.
