29 CFR 1910.212(a)(5): Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch. Armament shop; on or about 03 December 2014 and times prior; a ceiling mounted Sterling unit heater, labeled 11-05, The periphery fan blades were not guarded with protective barrier one-half inch or less. The measured distance from the floor to blade was 66 inches. This condition exposed employees to a caught in hazard.

29 CFR 1910.303(b)(7)(iv): Electrical equipment had damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating. Hangar 18 East bay; on or about 03 December 2014 and times prior thereto; a 50 foot electrical extension cord had damage to the protective outer jacket. This condition exposed employees to an electrical shock hazard.
