29 CFR 1910.178(p)(1): When a powered industrial truck was found to be in need of repair, defective, or was in any way unsafe, the truck was not taken out of service until it had been restored to safe operating condition: a) Job site, 629 Smithfield Street, Pittsburgh, PA: On or about December 3, 2014, during construction activities, the employer did not ensure that the Toyota forklift, serial number 66048, was taken out of service when repairs were needed. Deficiencies noted during the inspection included, but were not limited to, missing foot pedal covers, inoperative lights, lever labels that were not legibly marked with activating directions and a seat that was torn/damaged.
