29 CFR 1960.8 (a): The head of each agency did not furnish to each employee employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being locked into walk-in freezers:    (a) The walk-in freezer for frozen chickens next to the meat cutting area: Handle and rod mechanisms were installed on the exterior of three freezer doors that were about 4.5 feet wide by 6.66 feet high.  The mechanisms can be turned to insert the rods into eye bolts located on the freezers exterior.  The door mechanisms that allowed the doors to be opened from the interior were defeated by the handle and rod mechanisms.  Employees were exposed to the hazard of being locked into a walk-in freezer that was about 7 feet high by 12 feet wide by 12 feet long at a temperature of about 22 degrees Fahrenheit.      Among other methods, one feasible and acceptable method to correct this hazard is to:     Remove the handle and rod mechanisms and eyebolts and ensure door handle mechanisms are installed that comply with NSF/ANSI International Standard for Food Equipment - Commercial refrigerators and freezers (NSF/ANSI 7 ? 2009).  NSF/ANSI 7 ? 2009 requires that walk-in and roll-in refrigerator and freezer doors shall be capable of being opened from the inside, even when locked.  (b) The walk-in freezer to chill meat next to the meat cutting area: The mechanisms to open two 3.5 foot by 6.5 foot freezer doors to the meat cutting area were installed incorrectly.  The door mechanisms that allowed the doors to be opened when locked were installed on the meat cutting area side of the door instead of the walk-in freezer side of the door.    In addition, a handle and rod mechanism was installed on the meat cutting area side on one freezer door.  The mechanisms can be turned to insert the rods into eye bolts located on the meat cutting side of the freezer exterior.    A padlock and chains were installed on the meat cutting area side of the other freezer door.    Employees were exposed to the hazard of being locked into a walk-in freezer that was about 9.5 feet high by 30 feet wide by 36 feet long at a temperature of about 36 degrees Fahrenheit.    Among other methods, one feasible and acceptable method to correct this hazard is to:     Change the door mechanisms that allow the door to be opened when locked to the walk-in freezer side of the door instead of the meat cutting area side of the door; remove the handle and rod mechanisms and eyebolts; remove the padlock and chains; and ensure door handle mechanisms are installed that comply with NSF/ANSI International Standard for Food Equipment - Commercial refrigerators and freezers (NSF/ANSI 7 ? 2009).  NSF/ANSI 7 ? 2009 requires that walk-in and roll-in refrigerator and freezer doors shall be capable of being opened from the inside, even when locked.  (c) The walk-in freezer to chill meat next to the meat cutting area: The 5 foot wide by 7 foot high door to the loading dock area had chains and a padlock on the loading dock side.  The door mechanism that allowed the door to be opened from the interior can be defeated by padlock and chains.  Employees were exposed to the hazard of being locked into a walk-in freezer that was about 9.5 feet high by 30 feet wide by 36 feet long at a temperature of about 36 degrees Fahrenheit.  Among other methods, one feasible and acceptable method to correct this hazard is to:     Remove the chains and padlock from the door and ensure door handle mechanisms are installed that comply with NSF/ANSI International Standard for Food Equipment - Commercial refrigerators and freezers (NSF/ANSI 7 ? 2009).  NSF/ANSI 7 ? 2009 requires that walk-in and roll-in refrigerator and freezer doors shall be capable of being opened from the inside, even when locked.  (d) The Stay Gold Mfg., Inc., walk-in freezer, model number F, serial number 62-519, at the rear of the commissary: Chains and padlocks were installed on the exterior of two freezer doors that were about 3.33 feet wide by 7 feet high.  The freezer doors led to the loading dock area.  The door mechanisms that allowed the doors to be opened from the interior can be defeated by padlocks and chains.  Employees were exposed to the hazard of being locked into a walk-in freezer that was about 7 feet high by 18 feet wide by 40 feet long at a temperature of about 8 degrees Fahrenheit.    Among other methods, one feasible and acceptable method to correct this hazard is to:     Remove the chains and padlocks from the doors and ensure door handle mechanisms are installed that comply with NSF/ANSI International Standard for Food Equipment - Commercial refrigerators and freezers (NSF/ANSI 7 ? 2009).  NSF/ANSI 7 ? 2009 requires that walk-in and roll-in refrigerator and freezer doors shall be capable of being opened from the inside, even when locked.  THE DEFENSE COMMISSARY AGENCY WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1960.8(A) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 980306, CITATION NUMBER 1, ITEM NUMBER 5, AND WAS AFFIRMED AS A FINAL ORDER ON SEPTEMBER 4, 2014, WITH RESPECT TO A WORKPLACE LOCATED AT BUILDING S-182 APACHE ROAD, FORT HUNTER LIGGETT, CALIFORNIA.
