29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On or about 09 December 2014, two(2) employees without head protection were installing edge metal on a building ledge with an overhang approximately 3 feet above, exposing the employees to the hazard of striking their heads on the overhang.

29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person.  On or about 09 December 2014, Two(2) roofers were installing drip metal on the roof edge of a restaurant under construction up to ~24 feet high without the scaffold being designed by a qualified person, exposing employees to a fall hazard.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.  On or about 09 December 2014, two(2) employees performing roofing activities approximately 24 feet above the ground were wearing harnesses connected by lanyard and rope to anchorage points consisting of 2" x 2" pieces of wood, exposing employees to a fall hazard.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level.  On or about 09 December 2014, two(2) employees were performing roofing activities at a height of 24 feet, 8 inches where their personal fall arrest systems were attached to rope and the other to 2, 6 foot lanyards, exposing the employees to the hazard of free falling more than 6 ft.

29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were made without the manufacturer's written approval.  On or about 09 December 2014, two(2) roofers were installing drip metal on the roof edge of a restaurant under construction.  The two(2) roofers were transported horizontally and vertically up to ~24 feet on an unapproved personnel work platform, exposing employees to a fall hazard.
