29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  (a) On or about 12/09/2014 - Milling machine area, the employee operating a Supermax milling machine was exposed to occupational noise at 127.0% of the allowable 8-hour time-weighted average of 85 dBA or an equivalent sound level of 91.7 dBA. The sampling was performed for 463 minutes on 12/10/2014. Zero exposure is assumed for the remainder of the shift (17 minutes).
