(b) On or about 10/23/2018 - Machine Shop, the Bragonzi Horizontal Boring Mill was not guarded. Madsen Machine Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1009538, citation number 1, item number 2 and was affirmed as a final order on 02/09/2015, with respect to a workplace located at 500 39th Street E, Tuscaloosa, AL 35405.
