29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form:   On or about December 9, 2014, and at times prior thereto, airborne dust exposures were not evaluated for employees who  were voluntarily using N95 dust masks while cutting jelutong wood and polystyrene moulding with circular saws for frame manufacturing.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:   On or about December 9, 2014, and at times prior thereto, employees operating circular saws for the cutting of wood were voluntarily using dust masks to protect them from nuisance dust.  They were not aware of the safety, health and hygiene factors that ensure the proper use of dust masks.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Acrylic Department:  On or about December 9, 2014, and at times prior thereto, a RYOBI 10" drill press was not equipped with a chip guard to protect employees against the hazards of being struck by flying projectiles.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:  a)    Canvas Area:  On or about December 9, 2014, and at times prior thereto, employees used compressed air at pressures approximately 37 psi for cleaning purposes without any safety relief devices, exposing employees to the hazards of being struck by flying projectiles.   b)    Saw Area:  On or about December 9, 2014, and at times prior thereto, employees used compressed air at pressures approximately 60 psi for cleaning purposes without any safety relief devices, exposing employees to the hazards of being struck by flying projectiles.
