29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:   Worksite located at 6805 South Yale. Tulsa, OK 74136. On or about December 04, 2014 and employee was observed working from a Genie S-60 aerial lift without the use of a fall protection harness and lanyard. The employee was exposed to a fall hazard of approximately 30 feet.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:   Worksite located at 6805 South Yale. Tulsa, OK 74136. On or about December 04, 2014 and at times prior thereto, an employee was exposed to impalement hazards while working from an aerial lift above unprotected reinforcing steel in a concrete wall. Uncapped rebar used as reinforcement in a concrete wall had an unprotected height of approximately 2 feet.
