An emergency eye wash was not provided to flush both eyes simultaneously and the body in case of an emergency, on or about December 10, 2014.

The employer did not conduct an assessment to determine the type of hazards located at the jobsite and the appropriate personal protective equipment to be utilized when working around the jobsite hazards, on or about December 10, 2014.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a) O & K Auto Care Corporation dba Crystal Car Wash - Stockbridge, GA  - (Back and front area) Employees are exposed to chemical splash hazards.  The employer did not require and ensure the use of eye protection when spraying and mixing corrosive chemicals to prevent splashes to the face and eyes. On or about December 10, 2014, eye protection was not required while using corrosive chemicals.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a) O & K Auto Care Corporation dba Crystal Car Wash - Stockbridge, GA - In the production area, employees were exposed to hazardous chemicals such as, but are not limited to, Grimebuster (contains sodium hydroxide-corrosive), Brake Away (contains sodium hydroxide-corrosive), Purple punch (contains 2-Butoxyethanol) , Prime Pak Foam Brite (contains phosphoric acid-corrosive),  Tire Magic (contains N-hexane and Methylcyclopentane), Prime Pak Drying Agent (contains Ammonia Chloride, 2-Butoxy Ethanol, and Acetic Acid), Simoniz Hot Wax, Prime Pak Clear Coat, and Lava Sealant Purple (contains petroleum hydrocarbons).  The employer had not developed a written hazard communication program, on or about December 10, 2014.
