29 CFR 1910.178(l)(3)(i)(M): The employer did not review the operating instructions, warnings, or precautions listed in the operator's manual for the types of vehicle that the employee is being trained to operate: (Reference 29 CFR 1926.602(d) for construction): a) Jobsite - On or about, and at times prior to December 11, 2014, employees were not trained per the manufacturers safe operating instructions to recognize the hazards associated with free rigging (i.e. suspended loads) while operating a Genie, Rough Terrain Telehandler, Model GTH-844; exposing employees to falling object and struck-by hazards.

(Reference 29 CFR 1926.602(d) construction activity): a) Jobsite - On or about and at times prior to December 11, 2014, the employer did not ensure employees who operated powered industrial truck(s) such as, but not limited to, a Genie, Rough Terrain Telehandler, Model GTH-844, were properly trained and evaluated in their performance once every three years, exposing employees to forklift accidents, ejection, crush-by and struck-by hazards.
