29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On or about December 12, 2015, at 939 S. Jefferson Davis Pkwy., New Orleans, LA, the employer failed to ensure that flexible cords had adequate strain relief. Instance 1) Employees were allowed to utilize an extension cord that did not have adequate strain relief as a power source for a Makita Drill. Instance 2) Employees were allowed to utilize an extension cord that did not have adequate strain relief as a power source for a Makita Circular Saw.
