On or about March 14, 2022, an employee performing grinding tasks was using a damaged extension cord that was "daisy chained" to an additional damaged extension cord to power a Dewalt angle grinder on the south side of the site.

29 CFR 1926.453(b)(2)(v): A body belt with a lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift. On or about March 30, 2023, employees were performing concrete/form installation from an aerial lift without fall protection in several locations. Both aerial lift were in the same area next to each other. 1. The 1st aerial lift was a blue Genie Z-45 ID# 14470BLA. Two employees were in the lift without proper fall protection. 2. The 2nd aerial left was an orange Snorkel 660SJ ID#215418AH. Two employees in the lift without fall protection.
