29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:   a) In the Wood Pallet Repair Area, on or about December 17, 2014, a hearing conservation program was not implemented for an employee that performed wood pallet repairs who was exposed to noise at 152.02% of the action level (8-hour time-weighted average sound level of 85 dBA) or an equivalent sound level of 88.0 dBA during the 419 minute sampling period, resulting in a noise hazard.      b) In the Wood Pallet Repair Area, on or about December 17, 2014, a hearing conservation program was not implemented for an employee that performed wood pallet repairs who was exposed to noise at 190.88% of the action level (8-hour time-weighted average sound level of 85 dBA) or an equivalent sound level of 89.6 dBA during the 409 minute sampling period, resulting in a noise hazard.

29 CFR 1910.110(b)(18): 29 CFR 1910.110(b)(18): Fixed electrical equipment in classified areas. Fixed electrical equipment and wiring installed within classified areas specified in Table H-28 did not comply with Table H-28 and was not installed in accordance with subpart S of this part. This provision does not apply to fixed electrical equipment at residential or commercial installations of LP-Gas systems or to systems covered by paragraph (e) or (g) of this section.  a) At the propane dispensing station, on or about and at times prior to December 11, 2014, the electric pump that was located adjacent to an above ground liquefied petroleum (LP) gas tank that was used to refill portable LP tanks for the forklifts at a Class I, Division 1 location was not installed correctly, in that the wiring was not located inside sealed conduit, exposing employees to a fire hazard.
