29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a).  At the Golfside Villas building # 600 , employees performing roof repairs were exposed to a fall hazard of 17 feet from the edge of the flat roof to the ground below, on or about December 15, 2014.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  (a).  At the Golfside Villas building 600 the extension ladder used to gain access where repairs were being done on a flat roof was not positioned with its side rails 3 feet above the roof's landing exposing the employees to a 15-foot fall hazard, on or about December 15, 2014.
