(a) Bldg 3490, Wood Shop: On or about 10 December 2014 and times prior thereto, The Delta VBS 1220M, Vertical Band Saw, serial number 88809619, did not have energy isolation procedures for performing maintenance repairs on the machinery. This condition exposed employees to a caught in and amputation hazard. (b) Bldg 3490, Welding shop: On or about 10 December 2014 and times prior thereto, The Jet Horizontal Band Saw did not have energy isolation procedures for performing maintenance repairs on the machinery. This condition exposed employees to a caught in and amputation hazard.
