Bldg 3490, Room 122, Fabric Shop:  On or about 10 December 2014 and times prior thereto, the 8 foot metal ladder was completely painted covering safety information on the ladder.  This condition exposed employees to a fall hazard.

29 CFR 1910.37(a)(2): Exit routes must be arranged so that employees will not have to travel toward a high hazard area, unless the path of travel is effectively shielded from the high hazard area by suitable partitions or other physical barriers.  Bldg 3490, Welding Shop:  On or about 10 December 2014 and times prior thereto,  one cylinder of compressed oxygen gas and two cylinders of pressurized nitrogen gas located adjacent to the emergency exit door.  This condition exposed employees to delays in exiting the building resulting in smoke inhalation and/or fire hazards.

This condition exposed employees to smoke inhalation and/or fire hazards from delays in exiting the building.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.   (a) Bldg 3490, Wood Shop:  On or about 10 December 2014 and times prior thereto, The Delta VBS 1220M, Vertical Band Saw, serial number 88809619, did not have energy isolation procedures for performing maintenance repairs on the machinery.  This condition exposed employees to a caught in and amputation hazard.  (b) Bldg 3490, Welding shop:  On or about 10 December 2014 and times prior thereto, The Jet Horizontal Band Saw did not have energy isolation procedures for performing maintenance repairs on the machinery.  This condition exposed employees to a caught in and amputation hazard.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries.   Bldg 3490, Welding shop:  On or about 10 December 2014 and times prior thereto, a Class D Combustible Metal Fire extinguisher was blocked from access.  This condition exposed employees to delays in extinguishing welding fires during welding operations resulting in smoke inhalation and/or fire hazards.

29 CFR 1910.305(g)(1)(iv)(C): Flexible cords and cables were used where run through doorways, windows, or similar openings.   Bldg 3490, Fabric room:  on or about 10 December 2014 and times prior thereto, an 120 volt electrical extension cord was routed through the fabric shop to the seamstress working stations.  This condition exposed employees to an electrical shock and burn hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws.   Bldg 3490, Bay 10, South Bay area:  On or about 10 December 2014 and times prior thereto, a blue 15 foot extension cord had the insulation pulled away from the receptacle.  This condition exposed employees to an electrical shock and burn hazard.
