29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) Remer Lane Insurance Building - On or about December 15, 2014, an employee working on the ground level was exposed to head injuries from falling objects and materials such as, but not limited to, a broom that was thrown from the ground up to the roof.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a) Roof of Remer Lane Insurance Building - On or about December 15, 2015, the employer did not ensure that employees were adequately protected by personal fall arrest systems while walking/working at the edge of the flat roof of the building with sides that were unprotected by guardrail systems, exposing employees to fall hazards up to about 16 feet onto earthen ground and asphalt.
