29 CFR 1910.180(d)(3)(iii): Frequent Inspections were not conducted to identify whether safety devices were fully functioning.    a) Worksite: A Grove hydraulic truck crane, model AP-308, Serial Number 68746, was not equipped with seat belts and the horn was not functioning. Identification of safety devices were not part of the crane's monthly check list.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour.     a) Northeast end of the Laydown yard near the Oil Bay Area: An oxygen compressed gas cylinder was stored in direct contact with Acetylene and a propylene compressed gas cylinders; also several 5 gallon containers of oil and oily rags were within a few inches of the cylinders.

29 CFR 1915.152(a): The employer did not provide or ensure each affected employee with the appropriate personal protective equipment (PPE) for the eyes, face, head, extremities, torso, and respiratory system, including protective clothing, protective barriers, personal fall protection equipment, and life saving equipment, meeting the applicable provisions of Subpart I, wherever employees were exposed to work activity hazards that required the use of PPE.    a) Worksite: The employer did not ensure that employees were protected by PPE in the form of seat belts, while operating the Grove hydraulic truck crane.
