29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  On or about 12/16/2014, the employer did not ensure that written lock-out/tag-out procedures were specific to each of the machines in the Shredder Building and Production Plant.  Procedures did not include specific steps for each machine for a) shutting down, isolating, blocking, and securing the machine to control hazardous energy; b) placing, removing, and transferring of locks/tags; and c) testing the machine to verify that lock-out/tag-out was effective.

29 CFR 1910.157(e)(3): Portable fire extinguishers were not subjected to an annual maintenance check:  On or about 12/16/2014, in the Office Building, the employer did not ensure that the portable fire extinguishers were each subjected to an annual maintenance check.  The last annual maintenance check was performed on 09/30/2013.  Winnsboro Rubber Recycling, L.L.C. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 941018, citation number 01, item number 002 and was affirmed as a final order on 01/14/2014, with respect to a workplace located at 3000 Industrial Drive, Winnsboro, LA 71295.

29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck:   On or about 12/16/2014, the employer did not ensure that each forklift operator had successfully completed the training to be a certified powered industrial truck operator per the aforementioned training requirements prior to allowing them to operate a forklift.  Winnsboro Rubber Recycling, L.L.C. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 941018, citation number 01, item number 003, and was affirmed as a final order on 01/14/2014, with respect to a workplace located at 3000 Industrial Drive, Winnsboro, LA 71295.
