29 CFR 1910.29(a)(2)(v): Manually propelled mobile ladders - All exposed surfaces shall be free from sharp edges, burrs or other safety hazards. a. Warehouse/Aisle 41 - On or about December 18, 2014, employees were required to conduct warehouse operations with an unmarked "damaged" portable metal ladder ( Gillis - 10 Step). Employees were thereby exposed to falls from height. The damaged conditions were the following; 1. The diagonal metal bar (ladder's back support) was held together at the point of intersection with one metal pole/leg with adhesive tape. 2. One of the four metal poles/legs was broken, bent, and had some rust on it.

29 CFR 1910.176(b): Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse. a. Warehouse/Westside of the Building - On or about December 18, 2014, employees were required to conduct warehouse operations in an area were rubber tires were being stored loosely and at unlimited heights. Employees were thereby exposed to struck by hazards.
