29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following: a) Labcraft, Inc. located at 1070 Disher Dr. Waterville, OH 43566p On or about December 18, 2014, the employer did not assure employees were properly trained on the knowledge to safely change the blades of the Hyd-Mech horizontal band saws. Employees were required to change the blades on the saws without de-energizing the equipment.

Labcraft, Inc. located at 1070 Disher Dr. Waterville, OH 43566; On or about December 18, 2014, the employer did not ensure employees were protected while changing dies on the Cincinnati Press Brake. Employees are required to change the dies on the press brake and did not place a lock on the electrical disconnect, exposing them to hazards from accidental start up of the press brake.

Labcraft, Inc. located at 1070 Disher Dr. Waterville, OH 43566; On or about December 18, 2014, the employer did not ensure employees were protected while using the Hyd-Mech Bandsaws. Employees are required to use the saw to cut various metal parts and are exposed to amputation hazards from the unused portion of the band saw blade. b.) Labcraft, Inc. located at 1070 Disher Dr. Waterville, OH 43566; On or about December 18, 2014, the employer did not ensure employees were protected while using the Microcut milling machine. Employees are required to use the milling machine on various metal parts and are exposed to caught by hazards from the rotating chuck. c.) Labcraft, Inc. located at 1070 Disher Dr. Waterville, OH 43566; On or about December 18, 2014, the employer did not ensure employees were protected while using the Mori Seiki Lathe. Employees are required to use the lathe for various metal parts and are exposed to caught by hazards from the rotating chuck.

Employees were required to operate the shear to cut down various metal pieces and are exposed to amputation hazards in that the guard is not adjusted to ¼ above the part.
