29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) Labcraft, Inc dba Furn Tech Inc. on or about July 02, 2024, at the address of 1070 Disher Dr. Waterville, OH: The employer or employer representative failed to contact the Toledo Area OSHA Office within 24-hours after an employee received injuries resulting in an amputation from a workplace injury/incident that occurred on May 20, 2024. The employer reported the amputation incident to the Toledo Area OSHA Office via phone on June 25, 2024.
