29 CFR 1926.403(b)(2):  Listed, labeled, or certified equipment was not installed and/or used in accordance with instructions included in the listing, labeling, or certification:    50 Greenacre Ave., Longmeadow, MA: On or about December 18, 2014, the following electrical devices and/or equipment was not used in accordance with the labeling, listing or certification for that item:    a) A quad style junction box was used as shop made electrical multiple receptacle box to power tools at rear of this address.    b) A quad style junction box was used as shop made electrical multiple receptacle box to power tools on work platform supported by a Gradall lift at side of address.    c) A relocatable power tap (RTP) designed for indoor use only was used outside at the rear of this address to power various work tools.

29 CFR 1926.501(b)(1):Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) 50 Greenacre Ave., Longmeadow, MA: On or about December 18, 2014, an employee was observed walking on an open sided plank that was more than 9 feet above the ground below to reach his roof top work location, without any protection from falling to that lower level.   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms which were not specifically designed by the manufacturer for such use:    a) 50 Greenacre Ave., Longmeadow, MA: On or about December 18, 2014, an employee was observed working from an open sided platform that was held more than 9 feet above the ground  by a Gradall Material Handler.The warning label on the machine specifically stated, 'Do not use machine for lifting personnel...'.    La Rochelle Construction, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.451 (c)(2)(iv), which was contained in OSHA inspection number 313207888, citation number 1, item number 1a and was affirmed as a final order on January 10, 2011 with respect to a workplace located at 61 Abbey Memorial Dr., Chicopee, MA.
